Provinces such as Alberta, British Columbia, and Ontario also charge health premiums to supplement health funding. Some privately funded clinics do provide certain services with shorter wait times than the public system. Canadian physicians are not government employees. They retain a great deal of control over when, how and where they work. Instead of being paid salaries, most of them bill provincial governments on a fee-for-service basis. Canadian hospitals are also autonomous institutions that are generally, but not necessarily, governed by local health authorities.

Accessing Canada's health care system involves application for a provincial health card. Excluding inmates, the Canadian Armed Forces and certain members of the RCMP, the Canada Health Act requires all residents of a province or territory to be accepted for health coverage. Typically, the patients don’t receive a bill for hospital treatments or services provided by their doctor. Also the doctors cannot extra charge and provide basic medical services to patients who are willing to pay for faster or better service.
